During your first visit, feel free to come early and enjoy complimentary coffee, bottled water, and Wi-Fi in our comfortable waiting area. We will gather all your necessary information including a thorough review of your medical history and a New Patient form, if applicable. You can also fill out these forms online.

We proudly accept and file all PPO insurances. (We do not accept HMO, Medicare, or Medicaid)

We are also in network and preferred providers with Aetna, Blue Cross Blue Shield of Illinois, Cigna, Delta Dental Premier, GEHA, Guardian, Humana, Met Life, Principal, United Healthcare, Utnited Concordia Dental, Sun Life Financial, Maverest Dental Network, Ameritas, and Lincoln Financial Group.
